We would like to introduce our Vital Board Program available to our community. We have teamed up with Bell Hennessy Funeral Home in Williamstown to advance our program. Bell Hennessy invested into our already existed Medical History Forms. With a wonderful donation from them, we were able to create the Vital Board.

What is a Vital Board?

A Vital Board is a dry erase board that is placed onto the front of your refrigerator with the use of pre-attached magnets on the back of the board. On the front of our board (picture provided below) is a weekly calendar section, shopping list section, important numbers section, notes section, along with Bell Hennessy and Monroe Township Ambulance & Rescue information. Also attached to the front of the board is a dry erase marker. When flipped over to the back, you will see the vital information sections (sections for up to 2 people). You may fill in this information on the back in case one day you may need emergency medical services. Some information to write for example are is: Name, Date of Birth, Medical History, Living Will, Allergies, Medications and much more. Leaving this on your refrigerator will allow emergency personnel to obtain vital information in the matter of seconds.

How do emergency personnel know you are a participating resident?

Included with your vital board is a red sticker that is placed on your front door (picture provided below). This sticker when placed on your front door will allow emergency personnel to know you are a participating residence.

 

How much does it cost and how to I get one?

Our Vital Board Program is a free program to all residents. You may request a Vital Board by contacting our Organization President Anna Mills (founder of the Vital Board Program).  You may also stop at our building at 700 Corkery Lane in Williamstown (across from Milanos Pizza) on any Tuesday night from 7 pm – 9 pm.

You may request more than 1 set if your residence has more than 2 residents! And if you lose any part of your package, you may request a replacement with no cost to you.
